Initial Application for VTS Ophthalmology Credentialing

Please note that the application window will be open from January 1st, 2026 to February 28th, 2026. Before you begin the application, be sure to review the requirements in the bylaws to ensure you meet the necessary qualifications. The initial application is designed to confirm that you fulfill all the prerequisites for the credentialing process.

Required Documents for the Initial Application:

  • Work History/Hours: A detailed log of your experience in ophthalmology. Candidates must describe least three years (6,000 hours) of experience as a credentialed veterinary technician in ophthalmology within the last five years.

  • Proof of Technician License: Submit an image showing proof of your current license and the original credentialing date.

  • Proof of Current NAVTA Membership: Provide an image showing proof of your membership.

  • Continuing Education: Submit certificates or RACEtrack rosters for all continuing education hours (as per the bylaws), ensuring that all hours are completed before submitting your application.

  • Two Letters of Recommendation:

    • Option 1: Two letters from DACVO/DECVO.

    • Option 2: One letter from a DACVO/DECVO and one from a VTS (Ophthalmology).

    • Option 3: One letter from a DACVO/DECVO and one letter from a supporting veterinarian or a Diplomate from another veterinary college deemed appropriate by the Board of Regents (e.g., DACVIM, DACVS, or other).
      These letters can be submitted directly by the author to avot_vts@yahoo.com or included with your initial application.

Instructions for Applying:

  1. Review the Requirements: Carefully read the eligibility criteria and application requirements listed on the credentialing page. Ensure that you meet all prerequisites before proceeding.

  2. Prepare Your Documents:

    • Obtain and upload your two letters of recommendation.

    • Gather the required proof of your license, NAVTA membership, and continuing education (CE) hours. Ensure all CE certificates and/or RACEtrack rosters are included.

  3. Submit Your Application: Once all necessary documents are prepared, submit your completed application form with the required files attached.

Important Notes:

  • Incomplete applications will be rejected. Please ensure that you have provided all required documents before submission.

  • You will be contacted by the AVOT once your application has been received and reviewed.
